Independent Social Work & Support services

Independent Social Work Services & Case Management

We provide short and long-term independent social work and case management, supporting hospital discharge, care coordination, transitions, and statutory assessments. Our work helps individuals and families navigate complex systems with confidence.

We provide both short-term and longer-term social work support, including:

  • Hospital discharge coordination

  • Care brokerage

  • Transitions from residential care to supported living

  • Longer term social work input, coordinating support and care for individuals

We are specialists in assessments across the life course, including:

  • Care Act Assessments

  • Contributions to Education, Health and Care Plans (EHCPs)

  • Transition Assessments (Young people in transition to Adulthood, known as Preparing for Adulthood



Case Management

We work with individuals who may need support managing personal welfare issues on a longer-term basis, this is for a monthly fixed fee. We receive referrals from individuals and their families as well as solicitors and law firms.


Case management is a process to plan, seek, advocate for, and monitor services from different social services or health care organizations and staff on behalf of a client. The process enables social workers in an organization, or in different organizations, to coordinate their efforts to serve a given client through professional teamwork, thus expanding the range of needed services offered. Case management limits problems arising from fragmentation of services, staff turnover, and inadequate coordination among providers. Case management can occur within a single, large organization or within a community program that coordinates services among settings (Barker, 2003).


We take referrals directly from individuals and from organisations. The impact to the clients we support has been significant, ranging from organising holidays, family reunions, support with daily activities, moving home, better integration of health and social care outcomes, better health outcomes and timely interventions such as non elective surgery.
